Elle y rencontre et épouse Chris Sarandon dont elle divorce 15 ans plus tard, conservant néanmoins le nom de Sarandon en tant qu'actrice. Après la Convention des Démocrates en 1968, elle se rend à une audition pour le film Joe avec son mari Chris et, bien que celui-ci ne soit pas engagé, elle obtient le rôle principal (le film sort en salles en 1970). Le succès de ce film ne lui ouvre guère de portes, mais elle continue de tourner dans de petits films tels que Lovin' Molly (1974). En 1975 elle apparaît dans le film culte ''The Rocky Horror Picture Show''. Cette même année, elle donne la réplique à Robert Redford dans ''La Kermesse des aigles''. En 1980 elle est nominée pour l'Oscar de la meilleure actrice pour son rôle dans ''[[Atlantic City (film, 1980)|Atlantic City]]''. Mais il faut attendre 1988 pour qu'elle se fasse vraiment un nom grâce au film ''Duo à trois'' dans lequel elle rencontre l'acteur Tim Robbins qui est toujours son partenaire à cette date (2005).
